Eligibility Criteria
The project is available to individuals who are:
- aged 16 to 65 years
- committed and motivated to gain mainstream education, training or employment
- have a mental health need

Completing the form
Please provide as much detail as possible as this assists us to build a picture of your background and experience. Include information about any health care needs you have. A copy of an up to date CPA report, if you have one, would also be appreciated. Further information will be needed in meeting with the Vocational Support Advisor; this is to ensure that we can offer the best support possible.
If the referral meets the project eligibility criteria, you will be seen within three weeks for an informal information session and an action plan will be formulated with you within one month of the first appointment. The Care Co-ordinator and others involved in your care will be contacted during this period and kept informed whilst you are with the project.
If the referral does not meet the eligibility criteria for the service, you will be notified and provided with a written explanation of the decision.
Please indicate on the referral form if you have any specific needs around language or access.
Visiting the Project and Staff
You are welcome to visit the project and meet with a team member before you fill in the referral form. If you have any questions regarding the form or would like further information about the service, please do not hesitate to contact us.
